Add "Get Mail" checkbox for forums on subscribe.asp

Add a "Get Mail" checkbox for forums on subscribe.asp. This would allow the user to select which forums to subscribe to and which ones they would get mail from. Just like the subscribed threads.

Without the "Get Mail" option users will get an email from new posts to the forums they are subscribed to. On a busy forum this could be 100's or thousands per day. Since subscribe.asp provides the hooks for all the "MyForum" functionality on aspg one needs to choose forums on subscribe.asp in order to take advantage of the web based features of custom forum selection. However users who want these features now are forced to get emails which many might not opt for.
